Patient Insurance Process
The DETOUR Reimbursement Support Center (RSC) can provide support by helping to manage the insurance process. The RSC provides patient benefit investigation, pre-determination/prior authorization assistance, and reimbursement support.
STEP 1
Patient to Sign HIPAA Authorization
If the patient is currently in the office, have the patient sign and date this HIPAA Authorization form. Give the patient a copy for their records. You will need to submit the original form along with other documents.
If the patient is not in the office, they may sign the HIPAA authorization electronically by visiting the DETOUR Reimbursement Page and clicking “Sign your HIPAA authorization.” Once submitted, the form is automatically sent to the DETOUR Reimbursement Support Center. Please note that the additional documents listed below are still required.
STEP 2
Download and complete the Patient Enrollment Form
You will also need to gather the patients clinical notes and imaging (to establish medical necessity for DETOUR) and a copy of the patient’s insurance card(s).
